What is transfer of ADHD care?

Transfer of ADHD care means moving your ongoing ADHD care from the clinic that diagnosed you to Different Minds (ADHD ASD Norfolk). If you have been diagnosed elsewhere and have not yet been prescribed medication, we may be able to review your diagnosis and provide private medication care. We first check that your assessment meets UK clinical standards and that treatment is safe and appropriate for you.

A new assessment is not always needed

If your existing assessment is detailed and meets NICE, DSM-5 and UK clinical standards, we can often proceed after a review. If important information is missing, a further assessment may be recommended. We will explain the next steps and fees clearly before proceeding.

If you are already taking ADHD medication, please speak to our team about your circumstances before arranging a transfer.

What we need from you

To understand your diagnosis, health and treatment needs, we will usually ask for:

  • Your ADHD diagnostic report, including past and present symptoms, childhood evidence, the impact on daily life and consideration of other conditions
  • Details of any current or previous medication, doses, how well it worked and any side effects
  • Relevant physical and mental health history, including other diagnoses, and a recent patient summary from your GP
  • Proof of identity, such as a passport or driving licence
  • Up-to-date blood pressure, heart rate, height and weight measurements, where required

How the transfer works

  1. Talk to our team

Request a free pre-screening call to discuss your circumstances and the transfer pathway. We will explain the fees and agree payment arrangements by credit card or bank transfer.

  2. Send your documents

Provide your diagnostic report, relevant medical and treatment information, and proof of identity.

  3. Report review

Our clinical team reviews the reports from your previous diagnosis to check they meet the required standards and identify any further information needed.

  4. Hear the outcome

We let you know the result of the review and explain your options and any next steps.

  5. Agree your treatment plan

We agree a plan with you. If medication is appropriate, treatment can begin once all required checks are complete.

Safety comes first

We cannot prescribe medication until all required checks are complete. A transfer review does not guarantee that medication will be prescribed.

The titration fee does not include the medication itself. You purchase medication directly from a pharmacy. Medication supplies can fluctuate and availability cannot be guaranteed. We will discuss fees clearly before proceeding.

Starting medication and ongoing care

Titration means gradually adjusting medication to find the safest and most effective dose for you. It may take up to 3 months. Medication does not work for everyone and may be stopped if it is ineffective or poorly tolerated.

  • Your first 2 medication appointments must be attended in person at our Norwich clinic. Later appointments may take place by video.
  • You will need to provide blood pressure, pulse and weight measurements when requested.
  • You need 2 medication reviews each year, with additional prescriber contact if needed.
  • Prescribing may be paused if required reviews are missed or monitoring information is not provided.

Shared care with your GP

Once your medication dose is stable, we can request a shared care agreement with your NHS GP. If your GP accepts, they can issue prescriptions while we continue specialist oversight.

Shared care is not automatic

Your GP decides whether to accept a shared care agreement. Until an agreement is in place, your prescriptions remain private and chargeable.
